BURIED CLUES ARE THE DIFFERENCE BETWEEN WINNING AND LOSING

Knowing where to look when analyzing a client’s medication error claim is as important as knowing what to look for. As a practicing pharmacist in a variety of settings during my career, the one consistency I have seen is that the more a professionally trained practitioner of any kind has learned and the more letters he or she has following his or her name, the farther away from the patient and malpractice or negligence he or she is likely to be, and the less likely anyone is to find it.

FOR EXAMPLE...

For example, a "Pharmacist Consultant," (a misnomer, by the way), reviews pharmacy records once a month in facilities not required to keep an in-house pharmacy, such as nursing homes. A state requirement during monthly visits are "med passes," when licensed pharmacists are required to watch as attendants distribute medicines to the patients. These monthly visits bring fresh eyes to the process. Other In-house supervisors have no such oversight requirement so the pharmacists are the ones who catch mistakes.

Here, I have related some of the many experiences I have had which would bring tears of laughter if the consequences of the actions of those paid to care for others were not so BASIC to their well-being, indeed, on occasion, their very lives.
